Welcome to Fulton County Schools. To help make this transition a smooth one for both students and their families, it is important that you are informed of the requirements for new students entering the Fulton County School District.

In accordance with Fulton County Board of Education policy JBC and State of Georgia Department of Education Rule 160‐5‐ 1‐.28 Student Enrollment and Withdrawal, your child has been enrolled provisionally pending the receipt of the following required document(s):

(BCERT) Proof of Age (Must be in child's name): A Certified Copy of a Birth Certificate OR Hospital Issued Birth Record OR Hospital Issued Birth Certificate OR Military ID OR Valid Driver’s License OR Passport OR Adoption Record OR Religious Record Signed by an Authorized Religious Official OR Official School Transcript OR Affidavit as specified in Policy JBC (RVW) Proof of Residence Persons enrolling a student shall submit two proofs of residency from the school systems’ approved list of verifiable residency documents: one from the approved utility list and one from the approved residency list, upon initial enrollment in Fulton County Schools or upon entry into Kindergarten, 6th, 9th grades, and when there is any address change. (EEDN) Ear/Eye/Dental/Nutrition All students entering a Georgia school for the first time should have a Certificate of Vision, Hearing, Dental and Nutritional Screening. Certificates must be presented to school authorities, indicating that the student has received an ear, eye, dental and nutritional screening within the past year. The form must be completed by a physician/doctor, local health department and/or other health care professional or organization as outlined in the state guidelines for “Who Can Conduct the Screenings?” for Form 3300 ‐ Rev 2013. If a student does not have a certificate at the time of enrollment, these examinations must be completed within four months. The Georgia Department of Public Health requires that form GDPH 3300 be used for these screenings. This form can be obtained from the Fulton County Health Department or a private physician/dentist. (IMM) Immunization “All students must have a valid Georgia Certificate of Immunization (Form 3231) as required by law (Ga. Code 20‐ 2‐771). Georgia law allows for two types of exemptions from the immunization requirements: Medical and/or Religious, which requires the Affidavit of Religious Objection to Immunization, Department of Health Form 2208 to be signed and notarized. This certificate of immunization must be on a form provided by the Georgia Department of Human Resources (Form 3231) and completed by a Georgia Healthcare Provider.”
